Most people considering scalp micropigmentation have one question before everything else: what will it actually look like? Day by day, week by week — and how long until I see the real result? This guide answers all of it, honestly.
The Full Picture
Every client goes through the same process. Understanding each stage in advance removes the anxiety and lets you focus on the result.
Immediately after your first session, your scalp will look noticeably darker and more intense than the finished result will be. The dots appear sharp and defined. There may be mild redness or sensitivity — this settles within 24–48 hours for most clients.
This is not the finished result. It will change significantly over the next 10 days.
Aftercare Days 1–4: No washing, no swimming, no heavy sweating. Keep the scalp dry and avoid direct sunlight.
The scalp begins to heal. You may notice slight flaking as the outer skin layer naturally sheds — this is normal and is part of the process. The pigment starts to soften in appearance and the dots look less intense. The result already looks more natural and blended.
From day 5 onwards, light exercise is fine. Normal washing can resume from day 4.
Around day 10–14, most clients notice the pigment has faded considerably. The scalp can look almost as it did before treatment started. This is the stage that causes the most anxiety — and it is completely expected.
What is happening: the outer skin layer has fully shed and the pigment is now settled into the deeper dermis. The surface appearance is lighter. This is exactly why SMP requires three sessions.
If you message Mark at this stage, he will tell you the same thing: this is normal. Trust the process. Session 2 is about to change everything.
Session 2 takes place approximately 10–14 days after session 1. This is where the real transformation begins to take shape. Mark builds on the foundation from session 1 — adding density, refining the hairline, and adjusting the pigment depth to ensure consistent, lasting colour.
After session 2, most clients describe the result as already looking remarkably good. The same aftercare applies: 4 days dry, no swimming, no heavy sweating.
Session 3 is the finishing session. This is where the hairline is perfected, any uneven density is corrected, and the overall result is made completely symmetrical and natural-looking from all angles — straight on, side profile, under every lighting condition.
For most clients, session 3 is the moment they fully understand what SMP can do. The transformation is complete — now the healing begins one final time.
Same aftercare applies: 4 days dry and protected. After that, you're done — just maintain with SPF in strong sun.
Two weeks after your third session, the scalp is fully healed and the pigment has settled completely. This is your true, finished result. The dots have softened to a natural, hair-follicle-like appearance. The hairline sits exactly where it was designed.
This is the moment most clients take the photo they've been waiting for. The result looks like a naturally shaved head — not a treatment, not a tattoo. Just hair.
Over the first year, the result continues to mature naturally. The pigment softens very slightly and integrates further with your natural skin tone. For most clients, this makes the result look even more natural over time, not less.
Apply SPF 30+ to your scalp whenever in strong sunlight. This is the single most effective thing you can do to extend the life of your SMP and keep the result looking its best.
SMP is not permanent in the same way as a traditional tattoo — the pigment is formulated to fade gradually and naturally rather than turn blue or green over time. After 4–6 years, some clients notice the colour has lightened and book a single refresh session to restore full density.
Many clients go longer than 6 years without any top-up, particularly those who protect their scalp from UV. A refresh session is straightforward, quick and significantly less involved than the original three-session course.
Post-Treatment Care
The same aftercare applies after every session. Follow these four rules for the first 4 days after each treatment and your pigment will settle correctly.
After day 4, normal life resumes. Gentle washing, light exercise and most activities are all fine. From day 5 the scalp can get wet normally.
Long-term: apply SPF 30+ to your scalp in strong sun. That's the only ongoing maintenance SMP requires.

1. How manageable the process is. Most clients expect SMP to be more uncomfortable, more disruptive and more complicated than it turns out to be. Three sessions, four days aftercare each, and it's done.
2. The week-2 fade. Even after being told it will happen, clients are still surprised when they see the fade at day 10–14. This is the most common reason people message Mark during the process. The answer is always the same: it's normal, it's temporary, session 2 is the turning point.
